How to fill in your application
A short walkthrough of the MUV application form — what each part means, what to have ready, and the spots people usually trip on.
Have these ready before you start
- Your passport or national ID
- Your academic certificates & transcripts
- Proof of English proficiency
- A next-of-kin contact
Open it in Excel
The form is an Excel file with built-in dropdown menus. Open it in Microsoft Excel (or a compatible spreadsheet app) so the dropdowns work properly. Some of them — like the Visa Application Centre — only appear once you've made an earlier choice.
Filling in the form
Tell us what you want to study and when you'd like to start.
Your name, date and place of birth, gender, nationality, and passport/ID details. Everything marked * is required.
Your address, phone, and email. Make sure the email is one you check — this is how we'll reply to you.
List your education so far — higher education first, then secondary (SEC / MATSEC), then any other relevant skills or experience.
A contact person (usually a parent or guardian) — their name, address, email, and phone number. This is an emergency/contact reference only.
By signing, you agree to MUV processing your personal data for your studies. Add your signature and the date.
Documents to send with your form
- Scanned copy of your ID card / passport
- Certificates of your qualifications
- Academic transcripts
- Proof of English proficiency
Claiming Recognition of Prior Learning? Also include the RPL application form (AF-RPL20-1). Ask admissions if you're not sure whether this applies to you.
How to submit
- Opening the form in a basic viewer where the dropdowns don't work — use Excel.
- Picking a programme that isn't currently offered.
- Choosing a Visa Application Centre before selecting your country.
- A name that doesn't match your passport exactly.
- Forgetting to attach your scanned documents to the email.
